Community Care Coach (Bedford Choice Neighborhood)- Full-Time

As a proactive, encouraging, and creative professional with a “Whatever It Takes” attitude, the Community Care Coach is responsible for providing trauma-informed community development and coaching-based case management to Bedford Dwellings residents through a Trauma Informed Community Development (TICD) framework. TICD establishes and promotes resilient healing and healthy communities so that people can be healthy enough to sustain opportunity and realize their potential.           

 

The Community Care Coach with Macedonia Family and Community Enrichment Center will work with families to build trusting relationships, assess strengths and opportunities, problem-solve urgent needs, develop action plans for long-term goals, and provide information and referrals to community-based partners. The Community Care Coach plays an integral role in fostering, maintaining, and strengthening collaborative partnerships on behalf of residents. As part of an interdisciplinary team of Health, Education and Workforce Liaisons located on-site at Bedford Dwellings, the Community Care Coach will have the unique opportunity to support Bedford Dwellings residents to sustain opportunity during transformational redevelopment and revitalization.

What Will I do?

​​

  • Build trusting relationships with residents by consistently initiating outreach, responding promptly to questions, and addressing immediate needs.

  • Complete thorough coaching-based and person-centered assessment upon start of program.

  • Complete thorough reassessment at least annually, and update information at least quarterly.

  • Identify goals and develop action plans around education, health, and employment.

  • Provide resources and referrals to appropriate services and programs and follow-up to ensure successful connection.

  • Provide on-going service coordination to access complex systems and when necessary, accompany residents on calls, appointments, to meetings.

  • Practice flexible engagement by adjusting the level of support to residents according to their levels of need and interest.

  • Document efforts and outcomes in Client Path data system.

  • Maintain organized records for each family.

  • Attend certain Team meetings and Case Conferencing, Community meetings, and Resident meetings.

 

What will make you a great fit (the ideal candidate for this role has a unique blend of the following):

  • A passion for serving and empowering marginalized, socio-economic, diverse people groups

  • Bachelor’s degree from an accredited college or university in Psychology, Counseling, Social Work, Public Administration or related field is preferred but not required

  • 1-2 years’ experience full-time or equated professional experience in case management, community organizing, or community health programs

  • 1-2 years’ experience working with diverse agencies, community groups and socio-economic people groups

  • Good conflict management skills

  • Ability to work collaboratively with others

  • Excellent listening skills and ability to convey empathy

  • Must have reliable transportation

  • Ability to work independently and manage time 

  • Proficiency in the Google Suite and Microsoft Office Suite

  • Ability to provide Act 33, Act 34, and criminal background clearances.

 

SALARY:

$40,000 to $50,000 range, commensurate with experience.

All positions require satisfactory results of a background check and drug screen, as well as Act 33/34 and FBI fingerprint clearances. All employees will be subject to a probationary period of no less than 180 calendar days.
